Dalm is a Mac app that records a meeting — a call or a room — and drafts notes from the transcript. Get early access
A paper form works where a laptop gets in the way: a meeting across a table, a site visit, a workshop with a whiteboard, a talk where typing would look like you are not listening. The form keeps the order a reader needs later — who was there and why, what was said, what was decided, who does what — so the page still makes sense a month after the meeting. Everything fits on one sheet, with ruled lines sized for handwriting and a checkbox in front of each action, so the person who owns it can tick it off on the same page.

Open questions / next meeting
— Who keeps the old office keys until handover? — Next check-in: .
Two things in the example are worth copying. The last action has a dash in both columns: nobody said who orders the boxes or by when, so the form shows the gap instead of hiding it, and the next check-in is the place to close it. And the two decisions are full sentences with a day in them, so anyone reading the scan later knows exactly what was agreed.
How to fill it in by hand
Handwritten notes are only useful if someone else can read them after the meeting, so the form comes with four rules, the same ones printed at the foot of the page. They keep each sheet complete on its own: one meeting, the decisions written out, no invented owners or dates, and a copy stored where the rest of the meeting lives.
- One page per meeting. Start a new sheet for every meeting, even a short one, so a page never mixes two sets of decisions.
- Decisions as complete sentences. “Move on Saturday, 14th” is a decision; “move — Sat?” is a note that needs the writer to explain it.
- A dash when nothing was said. In Owner and Due, write a name and a date only if they were said in the meeting; otherwise put a dash, and raise it under open questions.
- Scan or photograph it afterwards. Take a photo or a scan of the page and store it with the meeting materials, so the paper copy is not the only one.
This form, the Word file or the full notes template
- This printable form — one page, filled in by hand, for a meeting where you write on paper rather than type.
- Meeting minutes and meeting notes templates — a fuller notes template to type, with a minutes template beside it, in Word and PDF.
- Meeting minutes template for Word — the formal record as a .docx to type, with a block per agenda item and lines to sign.
Every other template, from agendas to summaries, is on the templates page.
If you would rather not write it by hand
You can fill in this form by hand. If you would rather not take notes at all, Dalm is a Mac app that records a meeting — a call, or a conversation in the room through the Mac’s microphone — and drafts notes from the transcript. Your speech is recognised on your Mac, and no audio leaves it; what goes to our servers is the transcript, as text, and a language model writes the draft from that text. The draft is its own document rather than this form filled in: it edits as plain text and saves as PDF, Word (.docx) or Markdown, so it can be printed and kept next to the scanned pages.
How a meeting across the table is recorded on a Mac is on the page about in-person meetings. What each plan includes is on the pricing page.
Questions
- Is there a printable meeting notes template?
- Yes. This one fits on a single page: a header for the meeting, the date and the attendees, then the goal, ruled lines for notes, two numbered decisions, an action table with checkboxes and a place for open questions. It downloads as a PDF in US Letter or A4, with no sign-up, and prints in black and white.
- What should a meeting notes form include?
- The meeting and the date, who attended, the goal in one line, the notes, the decisions, and the action items with an owner and a due date. A place for open questions and the next meeting saves a second sheet. Anything more than that rarely gets filled in by hand, which is why this form stops at one page.
- How do I fill in meeting notes by hand?
- Use one page per meeting. Write the goal before the meeting starts, keep the notes short, and write each decision as a complete sentence. In the Owner and Due columns, write a name and a date only if they were said; otherwise put a dash. After the meeting, take a photo or scan and store it with the meeting materials.
- Should I print the A4 or the Letter version?
- Print the one that matches the paper in your printer: US Letter in the United States and Canada, A4 almost everywhere else. Both files hold the same form on one page. If your printer offers "Fit to page" or "Actual size", either works; scaling a Letter file onto A4 just leaves a slightly wider margin.
- Is the template free?
- Yes. Both PDFs download free, with no account and no e-mail address asked. The form is in English and carries no product name or branding, so you can print as many copies as you need, hand them out at a meeting or keep a stack on the table for the next one.
